Skateaway Adult Hockey Registration Form:
Winter 2011 Season: Starts January 9, 2010
Rosters & $100.00 deposit (not refundable) required by Friday December 31, 2010.
Total Money due $350 by your first game or you will have to forfeit.
Cost per team (10 players) $350 - $40 for each additional player. $10 for subs per game
Games played on Sunday 7-10 pm, Monday 6-10 pm and some Thursday’s (6-10 pm)
15 Regular Games and 2 round of playoffs.
Ref Fees $15 per game – paid to score keeper when you check in before the game starts.
All players can only be on 1 team in each division. Goalies can play for multiple teams.
All Players must sign waiver form after reading the rules and regulations before first game.
Skateaway will not be held responsible for any injury, fights, etc.
